2019-20 MPB Confirmation Interest Form
As Most Precious Blood continues to prepare children for the Sacraments of Initiation in the Restored Order in the 3rd Grade, the need for a traditional Confirmation preparation program is less desired. This form is meant to guide the Faith Formation Team in preparing an appropriate program for older Confirmation candidates who have already received their First Communion. This form does not serve as the registration for the preparation process for Confirmation, however we will use the data provided to send the link to register to those families who indicate their interest in preparing for the Sacrament of Confirmation during the 2019-20 academic year. If you have more than one child wishing to prepare for and celebrate the Sacrament of Confirmation this year, please fill out a form for each child.
