There are a few rules we must abide by in order to have a FUN and SAFE trip. Please read carefully!
Before you fill this out, you must confirm
with every member in your group.

at all times.
If you need another person to join your group, or if you are looking for a group, please let us know on this form so we can pair you up.
This form asks for student phone numbers. This is only so that your chaperone can contact you in case of an emergency.
