IN THE PARK WAIVER FORM
I
understand participation in LEGENDFIT! In the Park is voluntarily and I agree
to pursue such activities at my own health discretion. I release True Legends Foundation, LEGENDFIT!
and affiliates, staff, and volunteers from any and all claims of ANY
nature for personal injury or other damage or loss suffered by my child, by me,
or any family member/friend while involved in any activities associated with LEGENDFIT!
In the Park. I give consent to photograph, videotape, or otherwise digitally record and use images and/or sound recordings of
myself or my child or children (if applicable) to use in any public media, including radio, television,
internet, social media, print or in any of the organization’s or its partners’ publications,
productions, or posts. I understand that the intended use of such images and information is solely
for the purpose of advertising, marketing, fundraising and/or the promotional and public awareness
purposes for the organization. I hereby waive any rights or interest in the images or recordings, as
contemplated in this release.

¡Entiendo la participación en LEGENDFIT! Entrar al Parque es voluntario y acepto realizar dichas actividades según mi propia discreción de salud. ¡Lanzo True Legends Foundation, LEGENDFIT! y afiliados, personal y voluntarios de todos y cada uno de los reclamos de CUALQUIER naturaleza por lesiones personales u otros daños o pérdidas sufridas por mi hijo, por mí o por cualquier familiar/amigo mientras participo en cualquier actividad asociada con LEGENDFIT. En el parque. Doy mi consentimiento para fotografiar, grabar en video o grabar y usar digitalmente imágenes y/o grabaciones de sonido de mí o de mi hijo o hijos (si corresponde) para usar en cualquier medio público, incluida la radio, la televisión, Internet, las redes sociales, la prensa o en cualquiera de las publicaciones, producciones o publicaciones de la organización o sus socios. Entiendo que el uso previsto de dichas imágenes e información es únicamente para fines publicitarios, de marketing, de recaudación de fondos y/o de promoción y concientización pública para la organización. Por la presente renuncio a cualquier derecho o interés sobre las imágenes o grabaciones, tal como se contempla en este comunicado.
