Practice will begin 8/8/22 at the field across from the High School. Practices will be held 6-8 pm on Mondays, Tuesdays & Thursdays. Registration fee is $160 with a family cap of $300 for 2 or more siblings.

I understand that registration fee of $160 is due by first practice. Payment can be made by Venmo *MAKE SURE YOU NOTE "YOUTH REGISTRATION FEE" AND PLAYERS NAME. Payment can also be made by check made payable to Octorara Youth Football. NO refund will be issued after 1st game. Refunds prior to 1st game will be issued minus $50 fee. *

LIABILITY WAIVER: By submission of this form, I/We, the parent/guardian of above mentioned player (s), hereby give my/our approval for him/her to participate in any and all activities for Octorara Youth Football. I/We assume, all risks and hazards incidental to such participation including transportation to and from the activities; and I/We do hereby waive, absolve, indemnify and agree to hold harmless Octorara Football Club, Octorara Youth Football and all its affiliates including but not limited to: OASD, the organizers, sponsors, supervisors, volunteers, participants, officials and persons transporting my/our child to and from activities for any claim arising out of any injury to my child, whether the result of negligence or any other cause, except to the extent covered by accident or liability insurance. I/We know that participation in the Octorara Youth Football may result in serious injury to my/our child and that protective equipment commonly used by football players does not prevent all injuries to players. In case of emergency I/We authorize the above name participant to be treated by an available physician, medical staff person authorized by the Octorara Football Club or any of its member teams or any emergency response team. *
