All of the information I have provided is true and complete to the best of my knowledge. This animal will reside in my home as a pet. I will provide it with food, fresh water, shelter, affection, regular health care, annual physical examinations and vaccinations under the care of a licensed veterinarian. If adopting a cat, I will not have the animal declawed. DCAA prefers all cats be indoor only.  If adopting a dog, I will not have the animal’s ears or tail docked unless medically necessary and recommended by a licensed veterinarian. *
I am aware of my county/city requirements for maintaining rabies vaccinations and any pet licensing, tethering, and leash laws. I am prepared for the cost associated with age appropriate vaccinations, monthly heartworm preventative, flea/tick preventative, and any possible medical emergencies that may arise. *
I agree that if at any time I can no longer keep this pet, that I will notify and return the pet to Davidson County Animal Alliance. I will not attempt to re-home the animal on my own. *
I agree to take the pet to my personal veterinarian within 30 days of adoption. *
I understand that Davidson County Animal Alliance and/or their representatives are not liable for any future injury or damage caused by the animal. *
I understand that the pet’s prior history may not be known and that this pet may have been neglected, mistreated, or abused and that special care or precautions may be required.  I understand the pet may be shy, scared, or exuberant.  I understand what it means to be a rescue animal and will give the pet every opportunity to successfully become a member of my family.   *
I understand that this pet will need time to adjust to new routines, new family, and new surroundings.  This adjustment period may be anywhere from a few days to several months. I agree to carefully introduce the new pet to the existing dogs and cats. Coming to a new home is stressful for the new pet and your existing animals. Careful introduction is a must. Your new pet needs time and space to adjust to the surroundings and your existing animals need to feel they are not being replaced or crowded. I will not set up the new pet to fail by doing things like putting out high value treats or new toys. I agree to use crates and separation methods to introduce the new pet slowly and I do not expect instant best friends and happiness from the new pet and my current pets. I understand that relationships take time and I will give it that time. *
I am aware that animals available for adoption and rescue animals, as such, have often been exposed to a variety of diseases.  These disorders have an incubation period of up to several weeks. If the animal your adopt shows signs of illness at any point, please take it to your own vet for treatment. ** Once the animal is adopted the owner is responsible for all medical expenses.  DCAA is not responsible for medical care of an adopted animal.  If an emergency occurs, do not wait, consult your veterinarian immediately.   *
Any material and/or substantial breach of this agreement entitles Davidson County Animal Alliance to retake possession of the dog or cat. This agreement shall be binding upon any person in whose possession the dog may be found. *
