Welcome to Hazel Green Elementary! If this is the first child you will have enrolled in our school, we assure you that our staff will work diligently to ensure the experience your child has is second to none.

Please answer the following questions in order to provide the school with important student enrollment information.  Once your form has been submitted, please stop by the school and sign your child's printed form to verify that all information on the enrollment form is correct.

We will be contacting parents later this summer as to when the official start date will be, as well as the times for Open House.
