My name is Bernice Chan and I am a final year student of University of Birmingham (UOB). As part of my dissertation research, you are invited to participate in this UOB approved study of purchasing behaviour for ethical beauty products.

The purpose of this questionnaire is to understand your opinions, motivations and consumption of green beauty: cruelty-free, vegan and organic beauty products (cosmetics, skincare and personal care). Your participation will require your completion of this questionnaire which will take about 10 minutes of your time.

Your participation in this survey is voluntary and you may withdraw from the study at any point of time; even after agreeing to participate. For statistical accuracy, your identity and responses will remain anonymous. Data obtained will be strictly confidential and secured in a password-protected computer. The research will not publish or disclose your data, and will be destroyed upon completion in January 2020.
