Adult Behavioral Program Scholarship Application - Alyssa Burnett Adult Life Center
Thanks to the generosity of our community supporters, we are delighted to offer scholarships to participate in the behavior program. Candidates must complete the Adult Behavioral Program intake form (link to ABP intake form) and complete the intake process prior to being determined as eligible for aid. This is to determine eligibility for program placement and to ensure the candidate is suited for the treatment through the ABP.

Scholarships will offer aid for the initial intake appointment, and enrollment in a twelve-week behavioral program, with each session lasting one hour in length.  Appointments will take place at the Alyssa Burnett Center in Bothell.

Scholarship Application Process
After completing the intake application, families can then apply for aid through the scholarship application process. Please complete the scholarship application (link to smart sheet scholarship application form) to demonstrate your need for this service, and provide as much detail as possible about how a scholarship may benefit you/your adult.

The scholarship committee will meet weekly to review applications and will contact you directly if you are selected for aid.
 Eligibility Criteria
Scholarship applicants must be 18 years old, have completed the Adult Behavioral Program intake form and demonstrate financial need. Provide as much detail as possible to help convey financial need, and how aid would benefit the adult participant.

Once granted financial aid, if the adult misses 3 sessions, then program staff may need to suspend or terminate funding.
