Oregon Film Hiring Data Worksheet - Company Wide Aggregate Reporting
Oregon Film requires all incentivized projects to share specific hiring data with the office as part of our effort to identify and recognize the needs of all members of our community.  

This form can be used as an alternative to the Individual Hiring Data Worksheet that can be used for each individual employee to self-report upon the start of their employment. This form collects "company wide" data on overall numbers that have been self-reported to the Company. Individual data will not be shared publicly, only as part of aggregate reporting to better identify ways in which our programs can better serve our community as a whole.
