I would like to have my student participate in Video Game Club as an afterschool activity. I understand that there will be 10 sessions. Each session will run from 3:15-4:15 pm. Students are welcome to bring their own gaming equipment and games (E and E+ ratings only) but are responsible for their own equipment. The school will not be liable for lost or damaged student equipment. The cost for this activity is $100. The cost of this activity will be billed to my FACTS account as an incidental expense. *
