All families wishing to participate in our Children's Faith Formation Program should be registered and active parishioners at the Church of the Nativity, Menlo Park. Classes are for grades 1-8 including high school students who have not yet received the sacraments. If your child is preparing to receive the sacraments (First Reconciliation, First Communion, or Confirmation), they must be baptized and complete 2 years of preparation among other required activities. If not completed at the Church of the Nativity, then a letter confirming completion from your prior parish will be needed at the time of registration. A spot in our program is  always guaranteed. If this is your child's first year, a copy of your child's baptismal certificate should be sent to the DRE at time of registration. If you are a new family, you will be contacted by the Director of Religious Education to discuss your faith formation needs.

Media & Medical Release
Photos and video taken during our activities, programs & events demonstrate the work and accomplishments of participants as well as our parish. These photos and video may be used in the parish’s official communication channels such as the parish website, social media accounts, print publications, and e-mail newsletter. Occasionally, this content may also be used in submissions to local media outlets (e.g. Catholic San Francisco). Please note: a child’s name will never be used in association with their photo/video, unless specific permission is obtained from the parent/guardian. Do you give permission for photographs and video of your child to be used by Church of the Nativity, Menlo Park, in the parish's communication channels and in submissions to media outlets? *

Timely pick up of your child(ren) after events is requested. PLEASE drive slowly in the parking lot as there are child(ren) being picked up. The Church of the Nativity, Menlo Park, its directors, employees, volunteers, and representatives associated with the Faith Formation program have difficulty with excessive wait times for any child(ren) that does not have a ride home. I am aware that it is my responsibility to provide/arrange timely transportation to and from all Faith Formation events for my child(ren).

FAITH FORMATION: WE CAN'T DO IT WITHOUT YOU! Our program is dependent upon volunteers; please indicate your preference below. Check all boxes that apply. **All volunteers are required by the Archdiocese of San Francisco to have completed VIRTUS training (Protecting God's Children) and complete LIVESCAN fingerprinting. COST OF FAITH FORMATION AT CHURCH OF THE NATIVITY
Yearly tuition is $100 per child which covers the cost of supplies and books. **NOTE: Please do not hold up your family's registration because of financial issues. We are happy to make arrangements that work for your family. All information in confidential. Please contact Monica Hickam, mon.hickam@gmail.com if you need more information. Registration will be complete upon payment or other arrangements.
