This is the official application form to apply for Dr Cohn's Ageless Beauty Ambassador Team.

After submitting this application, our social media manager will email a confirmation.  YOU MUST REPLY TO THE CONFIRMATION AND ATTACH A RECENT PHOTO OF YOURSELF.

If you are selected as an Ambassador, you will receive 15% off all non-surgical treatments from Dr Cohn.  You will also be gifted a professional photoshoot with the other Ambassador team members to be used for promotional purposes.  You must agree to allow Cosmetic Surgical Arts and Rejuvenation of Oklahoma to photograph and video your procedures and testimonials to be used for online marketing purposes.
Ambassadors will be chosen by our selection committee and notified in early January.

It is very important to be able to commit to at least 3 of the 6 following dates for in-office treatments and for the final studio photoshoot for free individual and group professional photos.  Your treatment time slot will be between 1:00-3:00 on your choice of 3 of the following 5 dates.
