Nativity of our Lord is so glad you are part of the family! This application form enrolls you and/or your household members into one of Nativity's many formation programs - Catechesis of the Good Shepherd, Wednesday Night Faith Formation, RCIA, Marriage Preparation, etc. We know that it is tempting to only add one parents contact information to move more quickly through the registration, it is important for our records to accurately capture the whole family.
