Please complete the contact tracing form for the meet at WJMS on 11/5. Save the confirmation email and present that to the gate workers on Thursday to expedite entry into the meet.

This form will be cut off at 12:00 on Thursday. After that point paper forms will be available at the gate.

This form must be completed for ALL spectators. Parents can use their email and phone number for any children attending who are not in the race.

Spectators are required to keep 6 feet social distancing from others who are not in their household/group. Spectators should also keep 6 feet back from the course markings.
