If you have a service event opportunity to share with the society to receive extra credits as Event Coordinator, you may share that information in this form. To clarify, the board will NOT retroactively award credits; rather, it will either accept or deny credit before the event.

- Must notify the board via this form at least 10 DAYS before the event takes place
- If the event is approved, you will receive an email from the board notifying you that the event is approved and reminding you to fulfill the responsibilities of event coordinator.
