Please complete the form below for "new-to-district" students. If you have more than one student who is eligible for Pre K / Kinder, please complete a new form for each child.

An enrollment form will be sent to the mailing address provided along with a self addressed, stamped envelope for return.

Items needed at the time of registration are:
- Birth Certificate
- Social Security Card
- Immunization Record
- Proof of Residency
- Drivers License
- Proof of household income (PreK ONLY)
