Consent Form - Leaving designated collection point without an adult at the end of school day
Your consent is required for any situation where you wish your child to leave the supervision of their class teacher/designated member of staff at the end of the school day.  This includes pupils leaving their designated collection point to walk home or to meet a parent/carer at a different location such as The Hilt, Recreation Ground, Hiltingbury Infant School etc.

In order to give consent for your child to leave the designated collection point unaccompanied at the end of the school day, please read and return this form, in line with the school's procedures.

The consent form will be treated as an agreement between parent, child and school and by returning/signing it, you agree that:

* I have assessed the risks associated with the school route and my own child's confidence in leaving the designated collection point independently - including the need to follow current government social distancing guidance.

* I have discussed with my child the risks associated with leaving the designated collection point alone and strategies to minimise these risks.

* Should the school have any concerns regarding the arrangements for my child leaving the designated collection point, they will contact me to discuss this.

* Should there be any change to the arrangement for my child leaving the designated collection point, it is my responsibility to inform the school.

* I have read the temporary procedures for drop off and collection of pupils and have discussed with my child the importance of adhering to these procedures.

* I understand that by giving my consent for my child to leave the designated collection point, this is a permanent arrangement unless I notify the school otherwise.
