UTMUN 2024 Anonymous Equity Contact Form -- Delegate/Faculty Advisor
This form is intended for Delegates and Faculty Advisors to reach out regarding any Equity matters of concern/report any equity concerns they have experienced to the Chief Equity Officer, Harvi Karatha, and the Deputy Equity Officer, Iva Zivaljevic

This form has the option to include your name if you would like to be identified, but is not mandatory if you would like the submission to be anonymous. Please note that this form will not be collecting email addresses either, so submissions without names will be completely anonymous. All submissions will be kept *strictly confidential*, and will only be accessible by Harvi Karatha, Chief Equity Officer.
