此问卷调查是为了了解大家对食堂档位的卫生及服务态度等项目的满意度。问卷结果与分析将提供食堂管理委员会和档主们作为参考以改善之。如无光顾该档位,则无需填写,谢谢您的耐心。
评量标准如下:
a.服务态度(亲切有礼、友善且笑容可掬。)
b.色香味美(食物的色泽、口感、味道及美观)
c.健康卫生(食物少油糖盐、整齐的工作制服、档口卫生、食物卫生及餐具清洁。)
d.价格份量(是否物有所值,份量足够兼价格合理。)
This survey is to understand the hygiene standard and service performance of canteen stalls. The result and analysis of this survey will be provided to the canteen management committee and stall owners as reference for improvement. If you have never patronized a particular stall, you can leave the answer blank. Thank you for you patience.
The assessment criteria are as follow:
a. Service (Couteous, friendly and serve with a smile)
b. Palatability (the color and taste of the food served)
c. Health and hygiene (reduced oil, sugar and salt, cleanliness of stalls, tablewares and uniforms)
d. Price and quantity (price is reasonable while quantity is sufficient)
