I authorize and release Christ Church Episcopal Parish to use photographs, images, video and audio recordings of me, and any of my children listed below for the purposes of church publicity and communications, such as church publications and website, and for any other lawful uses as may be determined by Christ Church Episcopal Parish. This authorization includes use and publication of versions of all such photographs, images and recordings in any edited form and in any media format derived from the original. I further waive all rights to review or approve any uses of these photographs, images and recordings prior to publication in any media product or presentation. This release will continue in effect until such time as I provide written notice to Christ Church Episcopal Parish revoking this release. I have fully read and understand the terms of this release and attest that I have authority to make this release on behalf of any children listed below.
