In addition to this online form, the following must be received before the child's 1st day of attendance (for returning students) or at the intake meeting (new students) :
* Full tuition (paid in full or with monthly checking auto-withdrawal set-up)
* CHILD CARE ENROLLMENT AND HEALTH HISTORY AND EMERGENCY CARE PLAN (Prior to first day of attendance for all students as follows:)
-New students: new form
-Returning students: prior to first day of school, new form or parent can re-authorize form already on file if no changes
* Intake Under 2 (for students less than 2 years old - this must be updated every 3 months or with change in routine)
* MJP Intake Form (for new students only)
* Alternate Transportation Form (for students who get transported to/from MJP from/to MTSD only)
* PARENT/GUARDIAN REQUEST FOR NON-DAIRY MILK SUBSTITUTION FORM (for children with milk allergies only)
* NON-DISABILITY SPECIAL DIETARY NEED REQUEST FORM (for children needing other food substitutions only)
*HOUSEHOLD SIZE-INCOME RECORD (For eligible families only; see cover page of form for eligibility chart)

ADDITIONALLY: Within 3 months of 1st day of attendance: CHILD HEALTH REPORT & Immunization to be filled out & signed by doctor as follows: Each child under 2 years of age shall have an initial health examination not more than 6 months prior to nor later than 3 months after being admitted to the center and a follow-up health examination at least once every 6 months after admission. Except for a school-aged child, each child 2 years of age or older shall have an initial health examination not more than one year prior to nor later than 3 months after being admitted to a center and a follow-up health examination at least once every 2 years after admission. Most physicians have their own forms. If they ask for a form, this may be found on our website or ask Carol for one.
