Thank you for applying to be in the Lewisville ISD Collegiate Academy @ TCHS!  This form should be completed by the STUDENT using their LISD email account.  The application will take approximately 20-30 minutes and students will need the following information to complete the form:

*Student Email
*Parent/Guardian emails
*Highest completed level of parent/guardian education
*List of extracurricular activities
