All participants in Parks & Rec basketball's 2020-21 winter program are required to monitor and communicate their current health status to the director and/or Hopkinton Health Department. It is critical that all participants report even minimal symptoms and stay home when feeling ill, practice excellent hand hygiene, wear face coverings and enforce social distancing when possible. Thank you for completing this health and risk assessment.
