Simply Dance 2022 Show Registration and Health & Safety Form
Please answer the following questions on behalf of the child taking part in the show. Take note of any information points raised which will be needed on show day.

All information given and provided on this form is inline with and follows:

Charlotte Stripling and Brooke Bathgate all hold Full Chaperon Licenses, so any queries about child safe guarding and the current policy, please ask any of our safe guarding experts.

Each changing room will have 1:4 ratio (dependant on student/helper ratio) back stage helpers. All staff hold current Enhanced DBS checks.

Sign out Password
Please set your own safe password to ensure your children can be signed out at the end of each performance safely. Only you and any person responsible for the collecting your children can know this password. We will ask for this password on the door at the end of each performance. Children will not be handed over without this password being given correctly, by one of the names listed below.  Please take a note of your password before completing this form.

Do's and Don'ts
NO Nuts:  We have many students with allergies

NO Chocolate, Food or Drink which can cause a mess/stains on costumes, faces or hands.

NO Felt tip pen's

NO filming or photos to be taken back stage by students. Please make sure your children are aware of this prior to the event. The changing rooms will all have mirrors along with students who have photo restrictions, so for safe guarding purposes please ensure your children are aware that no photos are to be taken back stage.  

Please ensure children's full name is on all items of their belongings to avoid losing anything.

As the Audience, please remember no personal filming or photos are to be taken during any performance for safeguarding purposes. Anyone seen doing so will be removed from the theatre by stage staff.

Costumes required for each dance: If your child/children are in more than one dance, please bring costumes in a clear plastic box with child's full name on the front in clear capital letters - each costume needs to be put in to carrier bags with the dance name on the label stuck on the front of the bag e.g Harry Potter Ballet. This makes it perfectly clear to all our costume change helpers backstage  - which children’s stuff is which and what they need to wear. After performances, belongings can be put straight into the correct boxes ready for home.

Signing in and out
Please wait in the reception foyer at the end of each performance and your children will be handed back by our stage staff as each group are ready to go. We will call the child's name and ask who ever is collecting for the password given.

Fire safety Theatre Talk
There will be a fire safety talk given by the theatre staff to all children and helpers.
