[NCSC LI] Substantive Violation Form
Thank you for taking the time to file a substantive misconduct report at NCSC LI. We are incredibly sorry for your experience and would like you to know that we take all misconduct very seriously. This form will allow to you describe the substantive misconduct you experienced/are reporting on behalf of someone, as well as indicate what kind of action (or no action at all) you'd like the NCSC LI Secretariat to take. This form and its responses will ONLY be seen by the NCSC Executives (Secretary-General and Executive Director) unless you specify otherwise.

What constitutes substantive misconduct? (taken from our website page)
Substantive misconduct includes any plagiarism, pre-writing, or violations of a committee-specific technology policy (refer to your committee’s BG for more information on this), and can be reported HERE.

Other useful forms
This form should ONLY be used for the above substantive violations. While no misconduct report is too big or too small, we do want to remind you that to streamline the misconduct process, you can report ANY social/sexual misconduct HERE. You may also report general feedback about your committee/conference experience (again, nothing is too big or too small) at our conference feedback form HERE.

Providing contact information
A reminder that while most of these questions are optional in order to respect anonymity, we may require some further committee-specific and/or identifying information depending on what course of action you want to take moving forward. Unless you do not want ANY action to be taken, we ask that if comfortable, you include a form of contact so that the Executives/any other specified members of NCSC LI Secretariat can follow up with your submission.
