This form is for Bay Team members applying to have their dog agility judges training fees considered for scholarship reimbursement by the club.

Application must be in the board’s possession (submitted via this form) before midnight Dec 31 of the year in which the applicant is eligible for scholarship consideration.

Scholarship consideration shall only be given for training tuition and exam fees, not for travel, lodging, or any related expenses.

Awarding of and amount of scholarship shall be entirely at the board’s discretion, within the limit of the established scholarship fund. The board is not obligated to award the entire fund every year.

Eligibility Requirements:
1. Date of application submission must be no more than one year after the date of any of the requirements elements (training, exam passage, first assignment).
2. At the time of the application, applicant must be a current Bay Team member in good standing (dues paid).
3. At the time of any of the requirements elements (training, exam passage, first assignment), the applicant must have been a current member in good standing for at least one year.
4. The training, exam, and first assignment must be in a sanctioning organization for which the Bay Team is offering events in the year the application is made.
