Student Senator Affiliation & Information Form (Spring 2024 Term)
All Student Senators, whether elected or appointed, need to submit this affiliation form by the third meeting after their election or appointment, otherwise the seat may be declared vacant as provided by the Rules and Procedures.
It is important to fill in ALL of the information and to report any changes to the Senate Secretary through this form as soon as they occur.
