This form is to be completed for all absences. If you know in advance you cannot make a rehearsal or concert, fill out this form as early as you can. If you have to miss a rehearsal or concert less than 24 hours before the day, fill out this form as soon as you are able AND contact Kristine Han (Education Coordinator) via phone or email.
