Many things have changed this year so PLEASE READ EVERYTHING.  
This registration form will allow you to select workshops and meals.   When you complete the registration form and select submit, you will be emailed your invoice and a online payment link along with details regarding lodging.  If you need to change your registration after submission, please contact the Whitley County Extension Office at 606-549-1430 or by email to linda.burgard@uky.edu.  
Carefully review the following Refunds/Cancellation Policies for important dates regarding registration changes.  You will need to make your lodging reservations separately.  Lodging  Details will be in your registration email.

Refund / Cancellation Policy

Cancellations prior to December 5, 2022 are eligible for a full refund.  Please provide as much notice as possible as our instructors will be preparing for your attendance and in many cases purchasing materials and patterns.  Cancellations made after December 5, 2022 may result in the forfeiture of some (or all) funds.  We do not provide refunds or rebates for attendees who do not show up without prior notice or leave JQS early for any reason other than event cancellation by the JQS Committee.  If the JQS Committee determines the conference must be dismissed or cancelled, refunds or credits will be made to participants.  To initiate the refund, your cancellation needs to be communicated to Whitley County FCS Agent, Linda Burgard - 606-549-1430 or email linda.burgard@uky.edu.

For any cancellation, regardless of reason, it is YOUR responsibility to cancel your lodging reservation.  The policy implemented by hotels will vary depending on which hotel you select.
