Community Engagement Survey
Community engagement is an important indicator of congregational health.  In addition to helping FUUSN promote our goals as a Society, many people find that participating in the life of our church is highly rewarding. The purpose of this survey is to gauge ongoing activity levels and also to invite participation for those who may be interested.

Although individual responses from this survey will not be published, this is not intended to be an anonymous form.  Our goal is to find ways of promoting meaningful connections and involvement with FUUSN.

This survey is entirely voluntary; please feel free to leave any questions blank.
