Health Declaration Form
As a precautionary measure to reduce the risk of exposure to COVID-19, all members are required to complete a Health Declaration Form before attending CPD/PQSL events at the HKIS Surveyors Learning Centre (SLC).     If you fail to complete the form or answer YES to one or more of the questions, you will not be allowed to enter the SLC.

Non-members are advised not to attend CPD/PQSL events at the SLC if they have been travelling abroad, or have been in direct contact with someone who has tested positive for COVID-19, or have any symptoms of COVID-19 (including but not limited to fever, cough, shortness of breath, breathing difficulty and/or sore throat) in the last 14 days, or have been confirmed with COVID-19 disease.
