1.  A BONAFIDE Filipino Citizens who resides in Bamban, Tarlac;
2. Must be 5 feet and 4 inches and above in height;
3. Must be single without child/children;
4. Must be between 17 to 25 years old;
5. Has NEVER won any major title in any Provincial, Regional, National or International Pageants;
6. Not have competed on behalf of Bamban in any Pageant;
7. Will have to secure a written consent from respective guiardian/s;
8. Must be able to attend all scheduled activities and rehearsals;
9. Must complete and submit all requirement and application form ( please bring with you a 3R closed up picture and 5R whole body shot)
10. Must be endorsed by the Barangay Captain
11. Must be vaccinated

By accomplishing this form, I hereby agree to faithfully abide by them as well as other terms and conditions on my participation.  I hereby allow Mutya ning Bamban 2023 to execute a background check if necessary. I further certify that all the information I  provide in connection with my application and in connection with my participation in the Pageant is true and correct and complete in all respects.  By applying to participate as a contestant in the Pageant and other events, I agree that Mutya ning Bamban 2023 – production, its employees, agents, volunteers, vendors, sponsors, judges and representatives may use, transmit, broadcast, sell and distribute any information that I provide in connection with my application and participation in the Pageant, including any photographs and other images in whatever form provided, for any purpose in connection with the promotions, marketing, broadcast and performance of the pageant and related events.  I also understand and agree that my personal information including my image may be broadcast on television and through other forms of media and communication throughout the world.  I hereby represent and affirm that I am a bona fide Filipino citizen and I have not been convicted of any crime, whether designated a felony or misdemeanor, in any Region of the Philippines or in any foreign countries.  I understand that my participations in the Pageant is subject satisfying all conditions for acceptance and participation as contestant set forth in the Rules and Regulations.  I further understand and agree that the decision on my acceptance as a contestant will be made by Mutya ning Bamban 2023 – production in its sole and unfettered discretion and not subject to challenge or appeal.  I agree that I shall not disgrace, defame, libel or slander Mutya ning Bamban 2023- productions, sponsors, staff, volunteers, vendors , audience and committee members or anyone connected with the pageant.  I understand that my obligations commence upon signing the contract and remain in effect before, during and after the pageant.  My failure to comply with the requirements and the rules and regulations of Mutya ning Bamban 2023.  Pageant shall subject me to disqualification from the pageant and forfeiture of any titles, gifts and prizes associated therewith.
