It is useful for publicizing future events to use photos taken during camp. That said, we respect the wishes of anyone to not have their child's image used for these purposes. Please check one option below:

I grant the instructors and Rhizome DC permission to use photographs of my chilld (without naming the child) for any lawful purpose including publicity.

Every reasonable precaution will be provided for the safety of the children while participating in camp activities.

I acknowledge that neither the instructor, nor Rhizome DC are liable in the event of accident or injury to my child.

Covid-19 Safety Agreement *

Every reasonable precaution will be provided for the safety of the children while participating in camp activities.

I agree to abide by covid safety measues as communicated by Rhizome DC, and to stress their importance to my child as a participant in these programs. At the time of writing these precautions include: mandatory masks, mandatory hand washing before and after eating, social distancing. I understand that guidance may evolve and agree to abide by Rhizome's covid safety protocols or withdraw from the program.

Covid-19 Contingency Plan *

If in-person camps have to be cancelled, I would like to participate in a remote version of the camp program. (Remote camps would be shorter in duration and at a reduced price.)

If in-person camps have to be cancelled, I would like to withdraw my registration and be refunded.

Payment *

Payment (or a payment arrangement with us) is necessary to reserve your spot. You will receive an invoice within a day or two of submitting this form. Please indicate how you plan to pay:
