The masks distributed via this form do not provide liquid barrier protection. They should not be used as substitutes for medical devices.

These masks are reusable and should be routinely washed by department members. They should be worn in conjunction with all other safety measures outlined in the University of Washington COVID-19 response.

09/22/2021 Update: Each request is fulfilled with an envelope that contains two masks. The blue masks come with twist ties for fitting the mask over the nose.

General sizing guideline: take a flat ruler and place it next to your nose. Measure from the bridge of your nose to the point of your chin. Choose Small/Medium if you measure LESS than 4.5” / 115mm. Choose Medium/Large if you measure MORE than 4.5” / 115mm. Substitutions may be made without notice based on availability.
