Maple Glen Holiday Assistance Form 2022-2023
If you are in need of assistance with items for the upcoming holidays, please complete this form. We will do our best to connect you with the Maple Glen Giving Tree program or other programs throughout Hamilton County.

Please do not request assistance if you are currently receiving support from other agencies for the holidays (Gleaners, Good Samaritan, other WWS schools, holiday assistance through WYAP, etc). Thank you!

This form needs to be filled out by  Friday, October 28th to be eligible for assistance. 

Once I receive your assistance request, I will reach out to gather information regarding sizes for clothing and basic needs.
