This form is to assess the financial impact to businesses in the Highway 89 Road Closure Impact Area stretching from the Turtle Rock Rock south to the Highway 4 and 89 intersection. Please provide as much detail as possible. This form will be utilized by the Inter-Agency Review Committee to provide financial assistance to those impacted by the Highway 89 Road Closure.  These forms will not be released to the public and all information contained herein will remain confidential. Due to the limited funds available and the severity of the impact of the Highway 89 Road Closure the Highway 89 Road Closure Assistance fund may not be able to fund your full request.
