Please note:
If you have not yet been seen by Dr. Chagnon, please continue to fill out this form & submit it; a prerequisite medical exam by Dr. Chagnon will be scheduled in conjunction with the Acoustic Voice Evaluation, and completes the full Voice Profile for Singers for a Voice Status Report.

  • The Medical Laryngoscopy/Stroboscopy Exam gets billed directly and is covered by any Canadian provincial healthcare insurance or McGill's International Health Insurance (BlueCross Medavie).

  • The Acoustic Voice Evaluation is free and absolutely no cost to you or your insurance.
  • If you are interested in receiving an Audiometry Exam, please email drchagnon@orlvocalex.com, but do note that an Audiometry Exam is not covered by insurance.
