Volunteer Application Form
This form is to be filled out annually by any individuals who are looking to volunteer their time for one or more of our school programs and/or activities.  These volunteers will be working directly with students, not so much for coordination of events (ex: parking, concession stand workers, etc.).  Volunteers only need to fill this form out once per year.  Please select all the activities for which you will volunteer this year.  Volunteers will be run through the district's Raptor System, references will be called and then they will be BOE approved.  After these three things take place, volunteers may begin working with our school programs and/or activities.

Volunteers at the Phoenix Central School District are expected to:
  • Sign in and out at the Main Office;
  • Wear their Photo ID Tag while in the school building or while serving in a volunteer capacity;
  • Maintain confidentiality regarding student records and communications;
  • Discuss any questions or concerns they have with the director, principal, individual in charge, etc. 
  • Abide by the District’s Code of Conduct and the Rules and Regulations of the School and Classroom in which they are volunteering.
