MPCA LifeTime Membership Application

The MPCA LifeTime Membership is for households who live within the borders of Mount Pleasant.

To qualify, Mount Pleasant Resident(s) must also be:
1) 65 years old or older,
2) have had 10 years of consecutive MPCA membership and
3) all members covered under the LifeTime Membership must reside at the same address.

PLEASE NOTE: A LifeTime Membership is free, however, proof is needed to check residency and duration. This form will be sent to the MPCA Hall Manager where your membership will be verified and manually entered. Please note our Hall Manager only works two days a week. Thank you.
