Dear Parent/Guardian -

The Athletic Department went to an all electronic format for required paperwork and physicals for the the 2023-24 school year. This paperwork is for Athletics, Marching Band, Chorale and all other Extra-curricular activities. Please use the following link to access the Google Form that will be used to collect this information:

Please note the following:

1. Parents/Guardians and Students should review and fully understand the information presented in sections 11 through 16 before submitting the form.

2. All required information must be provided before the form will allow you to submit.

3. This form must be submitted before the student will be allowed to participate in any activities over the summer or during the school year.

4. In addition to the form, a current physical must also be on file in the athletic office.

5. If you do not have a current physical on file, please submit a copy of the current physical in electronic

form to the following email: athleticphysicals@ivcschools.com

6. Due to being paperless, please do not give paper copies of the physical or other required paperwork to a coach or the athletic office- Paper copies will NOT be accepted.
