Lennox COVID-19 Parent Agreement/Acuerdo de Padres

I acknowledge that I have received and reviewed the District’s Covid-19 Reopening Plan which is available online at the following link. (Yo acepto que he recibido y repasado el Plan de Reapertura del Distrito sobre el Covid-19 cual está disponible vía el internet en el siguiente enlace.)

In order to prevent the spread of Covid-19 I agree to the following requirement and protocols: (En orden de prevenir la propagación del Covid-19 yo acuerdo de seguir los siguientes requerimientos y protocolos:)

1. My child will wear a face covering while at school. (Mi niño utilizará una cobertura facial mientras está en la escuela. )

2. I will conduct a daily home health screening of my child before coming to school and I will not send my child to school if they exhibit any of the following symptom: (Yo conduciré una revisión de la salud en el hogar de mi niño antes de ingresar a la escuela y no enviaré a mi niño a la escuela si ellos exhiben cualquier de los siguientes síntomas:)
 
      Cough(tos)      
      Shortness of breath(respiración corta)
      Sore Throat (dolor de garganta)
      Loss of taste or smell (perdida de sabor u olfato)
      Diarrhea (diarrea)
      Vomiting (vómito)
      Fever above 100.4 (fiebre más de 100.4)

3. My child will have their temperature taken on arrival at school and during the day if symptoms present themselves. (A mi niño le tomarán su temperatura al ingresar a la escuela y durante el día si los síntomas se presentan. )

4. If my child exhibits any Covid-19 symptoms while at school, I will be notified and expected to pick them up from school as soon as reasonably possible. ( Si mi niño exhibe cualquier síntoma de Covid-19 mientras que este en la escuela, a mi me notificarán y seré esperada de recogerlos de la escuela lo más pronto razonablemente posible.)

5. In the event my child has been exposed to anyone who has tested positive for Covid-19 in the last two weeks, I will not send my child to school and I will notify the school that the student has been kept home for this reason. Per health regulations exposure is defined as an individual who was within 6 feet of the infected person for a total of 15 minutes or more over a 24-hour period. (En el evento de que mi niño ha sido expuesto a  alguien quien ha dado positivo para el Covid-19 en las últimas dos semanas, yo no enviaré a mi niño a la escuela y yo notificaré a la escuela que el estudiante ha sido mantenido en el hogar por este motivo.  Por las regulaciones de la salud exposición es definida como un individuo quien estuvo dentro de 6 pies de la persona infectada por un total de 15 minutos o más sobre un periodo de 24 horas.)  

6. I understand that the district may direct my child to quarantine because they suspect my child of having Covid 19 or because they have been in close contact with someone with having Covid-19. If directed to quarantine, I understand and agree to the following: (Yo entiendo que el distrito puede dirigir a mi niño de ponerse en cuarentena por sospecha de que mi niño tenga Covid-19 o por el motivo de que ellos han estado en contacto cercano con alguien que tiene Covid-19.  Si son dirigidos a una cuarentena, yo entiendo y acuerdo a lo siguiente: )

• Student required to quarantine may not return to school until permitted to do so by the District in compliance
       with direction from the Los Angeles County public health order. (Un estudiante requerido de hacer una
       cuarentena no puede regresar a la escuela hasta ser permitido de regresar por el Distrito conforme con la
       dirección de la orden emitida por el departamento de salud pública del Condado de Los Ángeles.)

• Students who are required to quarantine but are well enough to participate will be expected to participate in
        distance learning. (Los estudiantes quienes son requeridos de ponerse en cuarentena pero están
        suficientes bien para participar serán esperados de participar en el aprendizaje a distancia.)

7. I understand that due to current public health regulations, visitors are not allowed on campus (Yo entiendo que debido a las regulaciones actuales de la salud pública, los visitantes no son permitidos en las instalaciones escolares.)
         When dropping off my child I will adhere to social distancing rules (Al dejar a mi niño en la escuela yo
                adheriré a las reglas del distanciamiento social)
          If I need to speak with my child's teacher or a member of the staff, I will reach out via phone or email (Si
                necesito hablar con el maestro de mi niño o un miembro del personal, yo me comunicaré vía teléfono o
                correo electrónico)

8. I understand that the district is mandated to maintain confidentiality of student and employee health information. I will not ask the school for information regarding the identity of quarantined or possible exposure cases. (Yo entiendo que el distrito es obligado de mantener la confidencialidad sobre la información de salud del estudiante y el empleado.  Yo no le preguntaré a la escuela por información con respecto a la identidad de los casos de la cuarentena o de las exposiciones posibles. )

* Required
