Complete this form weekly for Outreach Summer Feeding at Lawrence County Public Library. If you have not already registered, please go to our website and complete the correct form. The registration form MUST be completed and approved by library staff  before weekly meal bags can be ordered. These are week long (7 days) food bags that may contain peanut products. The site you register for is the ONLY site that you may pick up from. Any changes must be discussed with the Outreach Coordinator. If you register for the weekly Outreach bags, you may still pick up hot meals from the Library, but you will not be eligible for the weekend bags.
