Please complete this form if you would like to request Term Time Leave for your child or children.

Please note that it is not the school's policy to authorise holidays during term time. Requests are likely to be declined except in the most exceptional circumstances. If your child is off school for unauthorised Term Time Leave, both parents may be issued with a fine, by the Local Authority, of £60 per parent.
