Please fill out this form if your family would like to receive free meals during the Summer Meal Program, which runs through Thursday, August 20. **Please note: Please give at least 24 hours for new requests to be processed and added to our meal counts.**

All children 0-18 are eligible to receive meals; they do NOT have to be Holden students. The meals are 100% free.

Starting Tuesday, July 21, supper will ONLY be available for pick-up, breakfast/lunch will be available for pick-up or delivery to 3 locations.

Tuesdays & Thursdays
     Breakfast and lunch is available for pick-up 12-2pm at the Holden Elementary Cafeteria doors.
     Supper is available for pick-up 12-2pm at the HS kitchen door.

Consolidated locations/new times for evening pick ups:
     Enon Baptist Church, Pittsville 5-5:30pm
     Elm Commuter Lot, Kingsville 5:40-6:10pm
     Library, Holden 6:30-7pm

You no longer need to sign up for produce boxes. They are first-come, first-served on Tuesdays at the HS kitchen door beginning at 12pm. Any personally-identifiable information you share on this form will be kept confidential.
