NJSIAA COVID-19 Daily Pre-screening Questionnaire
As a result of the decrease in COVID-19 cases across our region, the district is pleased to announce it is transitioning from the use of a daily health questionnaire to a self-screening process.

Beginning Monday, June 14, 2021, we will no longer be using the JDHS Athletics Covid-19 daily health questionnaire. Prior to that date, all families must certify that they will be self-screening their child(ren) each morning prior to reporting to school. The district may re-instate the use of the questionnaire based on guidance from state and local health officials.

In order to facilitate this process we are asking parents to certify this change by logging in to the OnCourse Connect Parent Portal<https://oncourseconnect.com/> and acknowledge no later than June 11, 2021. The acknowledgement is now available, and families may complete the form at any time.

For your convenience, a copy of the statement as it appears on the parent portal is provided below.

By acknowledging this notification, as a parent, you are certifying that you will complete the daily self-screening for your child prior to arriving at school. The acknowledgment will apply to each child attending any in-person activities inclusive of extracurricular and athletic programs through August 31, 2021. Additional screening protocols may be utilized when necessary.

I understand it is my responsibility to screen my child for a temperature (</= 100.4) and any COVID like symptoms each day prior to leaving home and reporting to school. Signs and symptoms of COVID-19 in children may be similar to those of common viral respiratory infections or other childhood illnesses. Symptoms may appear 2-14 days after exposure to the virus and include the following: Fever or chills; Cough; Shortness of breath or difficulty breathing; Fatigue; Muscle or body aches; Headache; New loss of taste or smell; Sore throat; Congestion or runny nose; Nausea or vomiting; Diarrhea.

I understand it is my responsibility to monitor my child(ren) for symptoms, report any symptoms or possible COVID-19 exposure to the school nurse, and follow any protocols based on guidance from state and local health officials.

I understand it is my responsibility to keep my child(ren) at home if they have symptoms or meet exposure criteria as set by state and local health officials.

Please acknowledge no later than Friday, June 11, 2021. If we do not receive your acknowledgement, your child will not be permitted on school grounds.


As a reminder, families will still have to complete the daily screening through June 14, 2021, and students will not be allowed to participate in any in-person learning or extracurricular activities until the acknowledgement has been completed.
