Smart Motors, which is generously doing the safety inspections, needs your email address, first and last name, street address, city, state, zip and the VIN number. The Vehicle Identification Number (VIN) should be printed on your car's insurance card. It is also on a metal label that is visible through the lower driver's side corner of the windshield. Here's an example photo  https://bit.ly/vinlocation. We need all of this information to set your appointment time. Collecting this information now means that we do not have to collect it "at the door" thus letting us make more appointments available.    

Once you have filled in this form, click the "Submit" button and an email from "Triad & RSVP of Dane County" will be sent to you with the information you provided and the appointment time you chose. IF at some point after registering you need to cancel your appointment, please send an email to mthomsen@rsvpdane.org so that the time slot can be opened up and made available to somebody else.

At Smart Motors
Car Check will be using Express Lube Bay #6. The lighted number of the bay is above the bay door. We will have a 10'x10' Red & White Toyota pop-up tent in front of Bay #5 with our welcome table and a sign that says "Car Safety Check Event Registration". A Smart Motors employee will pull your vehicle into the service bay, and a team of technicians will proceed with the safety check and top off the fluids in your car (unless you instruct them not to do so). While the technicians check your car, you can get a cup of coffee or a bottle of water in Smart's Guest Lounge. You will be notified when the safety check on your car is finished. When you return to your car, a Smart Motors technician will give you a sheet outlining the items that were inspected and if any service is recommended. You will also receive a complimentary car wash code that you can use when it's convenient for you to have your car washed at one of Smart's automatic car washes.
