After you've filled out this form, the Director of Membership will provide the Institutional Membership Manager with access to a Google Spreadsheet. The Membership Manager should then update the spreadsheet with their roster.

Once payment, the completed form, and the roster are received, the Director of Membership will initiate the membership update process.
