C2C Grant Application Initiation      
Please note that this is part 1 of a 2-step process for applying for the C2C grant.  This form will alert your C2C Coach that you believe you meet the eligibility requirements for the grant and would like the Coach to initiate an application on your behalf.  When the Coach has initiated your grant application, you will receive an email indicating that it is time for you to complete step 2.  You have not applied until you have completed both steps.

This form must be submitted to your UM C2C coach EVERY semester in order to be considered for grant renewal.  Deadlines for applications are:

Fall - October 31st
Spring - March 31st
Summer - June 30th
