We’re inviting you to participate in a survey. Participation is completely voluntary. If you agree to participate now, you can always change your mind later. There are no negative consequences, whatever you decide.
Purpose: We want to better understand the needs of the people who are attending physical therapy at the APTA Alaska Physical Therapy Community Clinic. We are also looking for feedback on what we can improve.
Procedures: We will collect deidentified information with this survey. We plan to use this information in a presentation to help promote pro bono physical therapy services in other states.
Time Commitment: This survey will take approximately 5 minutes of your time.
Drawing: Upon completion, you will have the option to be entered into a drawing for one of two $25 Amazon gift cards.
