FY 22-23 Non-Academic Survey Parent Opt In
Due to HB 2567, USD 315 is required to receive parent consent for each non-academic survey during the school day that contains questions about personal and private attitudes, values, beliefs, or practices of the students or any of the student's families, friends or peers.  

Each survey administered through first semester is located on the district website under public information.

Each of the following non academic surveys will be administered between September 2022 and December 2022.

You will need to complete one non-academic opt in survey for each of the student(s) that you have enrolled in USD 315 in grades 3-12.

Based on the guidance of HB 2567, grades K-2 will not be completing any non-academic surveys during the school day that cotains questions about personal and private attitudes, values, beliefs, or practices of the students or any of the student's families, friends or peers.

Grades 6th, 8th, 10th and 12th Kansas Communities That Care:
Student Survey core questions – The survey contains questions pertaining to Communities That Care Risk and Protective Factors, including sections on Demographics & School Climate; Peer Influences; Drug/Alcohol/Tobacco Usage; Community-Based Perceptions.  The students will only be taking the Core Survey. The KCTC survey measures teen substance use, delinquency, and related problem behaviors in schools and communities. The survey gives us insight into the problems students face and shows what we can do to help them succeed. The information is important for planning effective prevention programs in our school and community and provides data to assist in applying for grant funding.

Grades 3-5 Project EVERS
The Student Engagement Instrument - Elementary (SEI-E™) (for students in grades 3-5) is a brief 31-item anonymous survey that measures student engagement in school. This survey is an important component of the evaluation of Project EVERS operated by the Northwest Kansas Educational Service Center (NKESC). The SEI-E is used with permission by the University of Minnesota.

SEI-E results are shared with Project EVERS districts each year (beginning in 2020) to assist with their efforts to best meet the educational needs of their students. Districts receive their own results disaggregated by concept and age/grade level. Since the survey is anonymous, there is no information that could identify a student. A project-wide summary of the data (non-identifiable by student/district/school) also is used to respond to the Government Performance and Reporting Act (GPRA) Measures in the Project EVERS Annual Performance Reports submitted to the U.S. Department of Education.

Grades 9-12 Kansas College and Career Compentency Surveys
Self-efficacy, Self Regulation, Conflict Management and Assertiveness assessments included in this suite are formative measures designed to guide students’ reflection and educators’ instruction. The assessments are not intended to provide a summative evaluation. When combined with other data sources, these assessments guide decision-making for direct instruction that builds
students’ knowledge, for guided practice that develops students’ fluency, and for independent practice with ongoing coaching that promotes students’ proficiency and generalization. All assessments are free for educational professionals to administer if utilizing the results for skill development or program improvement.
